Psychological Assessment and Testing Request

Please complete this request if you are looking to book for our psychological assessment and testing service. As these require different appointment lengths and number of appointments, we require you to let us know as much information as you can about what assessment and testing you are seeking. If you have request from your GP, Psychiatrist or Paediatrician, please attach the referral letter and any other documents.
